Victorian club permit scheme

Hi all
I have received the renewal for my Victorian club permit, and reading through some of the posts, it looks like we have multiple authorised signatories therefore can you please advise who I should send this to? If Clyde is this the correct postal address is: PO Box 8284 Carrum Downs 3201?

Could you please clarify - "Application forms (2 of them)". - does this mean 1 of each of the forms below ?
1. Vic Roads - Club Permit Application form
2. Vic Roads - Vehicle Eligibility and standards declaration for club permit vehicles

Club Permit Renewals / Rego Payment Online
I got a shock today when I rang VicRoads to enquire about my registration - I was told they had cancelled my Club Permit Rego as it hadn't been paid (May 2021). When I disputed, I was asked to prove when payment was made etc. Eventually, the operator passed the call to the Payment Section - another operator explained it was a different process to the normal car rego and it's a manual process to tick a box as PAID. After another 5 minutes when the operator put me on hold - by which time, I had found from my bank transactions from May 2021 it was paid. I would have been driving without a valid / paid Club Rego.
The operator admitted the error was due to someone NOT ticking the right box after I paid online !
